Process for New patients:
Once we receive your request either via telephone call, voicemail, or via website form you will receive via either Text or Email the New patient Intake and History forms. Medical Spa in Edmond OK, requires the Initial Free Consultation take place face to face in the office. We request you either bring the completed forms with you when you come in or complete them and either email them to Diane@drdavidjayne.com or fax them back to 405 330-0092. This will save a tremendous amount of time for you in our office.

If you are interested in starting BHRT (Bio-Identical Hormone Replacement ) you are generally NOT required to fast for those hormone levels and those may be able to be obtained at your first visit.
We appreciate your arrival a few minutes early as we may be running ahead of schedule and if you cannot bring you completed New Patient forms with you or send them in electronically we request you come in 30 minutes prior to your appointment so that Dr. Jayne can spend the time he needs with you at your Free Consultation.
Routine medication refills are handled during normal business hours Monday-Friday. Please call your pharmacy first as you may have refills remainIng on your prescription.
You should allow at least 30 minutes and up to an hour for your initial visit with David Jayne MD at Medical Spa in Edmond OK as he needs time to assess your specific issues and make appropriate recommendations for your unique circumstances.

Follow-up visits generally are shorter than the initial visit so 15-30 minutes are generally sufficient to plan.

It becomes a visit when it crosses over to treatment. Ordering lab is treatment because Dr. Jayne is required to go over the results and discuss possible treatment at another visit.
Established patients coming in for a lab draw only do not pay a visit fee if they do not see Dr. Jayne at that time but will need to schedule a lab follow-up with Dr. Jayne to discuss the results and any possible modifications in treatment and there will be a physician visit fee.
